Hours: 35 hours per week, working hours between 09.00-16.30pm  Monday – Friday

Rate of Pay: £22,812.79 per annum/£12.53 per hour. A mileage allowance of 45p per mile is also paid

Do you have line management skills, coupled with a desire to assist older people in accessing services across the City of Plymouth?

The primary purpose of this post is to undertake assessments for patients who are referred to our Discharge Service on discharge from Hospital. The assessments are carried out to establish what holistic support is needed with a focus on reablement and preventing readmission. This is a commissioned service and as such you will work closely with the Discharge Teams at Derriford, Mount Gould Hospital, DAU and Short-Term Care Centre as well as internal staff.

The role has shared joint line management responsibilities for the Rapid Responders who directly deliver the support.

About You

Successful individuals will have excellent communication skills, be compassionate, organised and highly motivated to provide a high level of service to both clients and stakeholders.

Previous experience in a similar role is desirable but training will be given. It is essential that the successful candidate has a clean driving licence and access to a vehicle for work purposes, and a willingness to take clients in their own vehicle to appointments. You will also require basic business insurance. A mileage allowance will be paid (45p per mile).
