Hours: 30 hours per week working Monday - Friday with availability between 9am – 5pm. 

Rate of Pay: £17,940 per annum/£11.50 per hour.

This post involves providing a commissioned service as agreed with the NHS at the point of discharge for individuals living in their own homes. It will involve a variety of different task which may include, collection of patients from hospital, helping them to get to appointments, basic food preparation, light cleaning, shopping and collecting prescriptions for them.

The post is 30 hours per week working Monday - Friday with availability between 9am – 5pm. This is initially a temporary contract until 31/03/2025, based on the needs of the service there could be a possibility to move to a rolling contract thereafter.

About You

Would you like to be part of a team helping to relieve the pressures on the NHS by supporting those leaving hospital?

Successful individuals will be compassionate, organised and highly motivated to provide high levels of support to clients, ensuring that those initial weeks post discharge pave the way to returning to living independently at home.

Previous experience in a similar role is desirable but full training will be given. It is essential that the successful candidate has a clean driving licence and access to a vehicle for work purposes, and a willingness to take clients in their own vehicle to appointments. You will also require basic business insurance. A mileage allowance will be paid (45p per mile).
