The Age UK Leeds Travel Connections service is part of a larger Travel Connections Programme that is being led by Leeds Older Peoples Forum across the city
The programme aims to:
• Support people aged 50+ to have more positive experiences of the transport system
• Feel more confident in making journeys
• Reduce isolation
• Improve social connections
• Increase access to services
• Identify key challenges to older people in accessing transport
Age UK Leeds' focus in the programme is to particularly work with, whilst not being limited to people aged 50+ with disabilities, including wheelchair users
What support will older people receive?
We work with the older person for up to 12 weeks, providing Travel Training
Travel Training will be provided through a series of goal setting conversations and practical support to make journeys
We will give older people an opportunity to discuss what is important to them about using transport reconnecting with their community
We will support through home visits and assistance to make journeys, working through agreed steps to reach agreed goals
The aim is that each older person will complete one independent journey

Service Criteria
We can accept referrals for older people aged 50+ from across Leeds but we will ask all older people who take part to agree to the following before support is given:
I commit to Travel Training for up to 12 weeks
I am able to leave home and would like to travel independently
I understand this is not a befriending service
I understand this is not for support over 12 weeks
Unfortunately this service is not suitable for older people with complex mental health. We are also not able to provide personal assistance and so older people using mobility aids or wheelchair must be able to use their mobility aids or wheelchair independently (wheelchairs must be electric or self propelling)
How to refer yourself or a family member
If you think that yourself or a family member would benefit from this service please get in contact to introduce yourself to our friendly team by calling 0113 389 3000 or email us. When we speak with you we will require consent to record your contact details and some key information in order to help us respond to your enquiry. If you are enquiring on behalf of a family member please ensure that you have their consent to share this information. We may ask to speak to the older person ourselves to obtain this.
How to make a referral as a professional
We can receive referrals directly from older people or from professionals, including other AUKL services
Referrals from other AUKL services or external organisations must be submitted using the AUKL Travel Connections Referral Form and email us

If you would like to discuss a referral before submitting a form please contact us on 0113 389 3000 or email us and one of the staff will contact you
What happens if we are not able to support you with this service
If we are not able to provide support from this service, we will discuss options for alternative AUKL services and other organisations as appropriate. We may provide signposting and information about these options or make a referral on your behalf with your consent.
