The Post Discharge Settling Service is a free service that supports vulnerable or mildly frail older people from the age of 55 who live alone in the community or do not have anyone who can provide them with adequate support or assistance.

How we can help

The Post Discharge Settling Service is a free service that supports vulnerable or mildly frail older people from the age of 55 who live alone in the community or do not have anyone who can provide them with adequate support or assistance. It is a ‘light-touch service’ and no personal care can be provided by our assistants. Our aim is to provide short interventions focussing on settling the client back into their home after discharge from hospital and aiming to prevent readmission. This will be done by providing companionship, conversation, reassurance, and confidence building to help patients adjust and as well as helping the patient identify and access further support that they might benefit from and signposting or referring them onwards.

The service is not a discharge dependent service and only operates between the hours of 9am – 5pm. Hospital staff/healthcare professionals can request this service for patients that they believe would benefit from assistance to regain their confidence and independence in their home environment with the aim to prevent readmission to hospital. We cannot guarantee same day support. We will endeavour to see patients as soon as we can but it will be dictated by a number of factors, including how many referrals we receive, and we will keep clients well informed.

Department or Urgent Care at Princess Royal University Hospital
- Hospital Staff at the Frailty Unit at Orpington Hospital
- The Transfer of Care Bureau
- Bromley Local Authority Staff, including GPs and Social Care Practitioners
