Our Out and About service is designed to support people to re-engage in their community following difficulties, illness or a crisis that has resulted in loneliness and / or isolation.

Please note that this service is now offered as part of our Independent Living Service.

What the service offers

Our Out and About service is designed to support people to re-engage in their community following difficulties, illness or a crisis that has resulted in loneliness and / or isolation. After an initial assessment with a support worker from the team will provide support for a 6-8 week period to help you achieve your personal goal – whether that’s meeting a friend for coffee, catching a bus into town, or linking you in with activities in your local area. The service is free of charge and operates Monday to Friday.

Making a referral

You can refer yourself directly by calling our Information and Advice service on 01904 634061, they can also give further information on the service if required.
Referrals can also be accepted from professionals or other charities working with older people - the preferred method of referral from these is by completion of a referral form. If you require a copy of the referral form or have any specific queries on the service contact 01904 726191.
