What is the service?

Practical support for a short time following a stay in hospital, supporting people to regain their confidence and independence.

What can you expect?

Support can include:
• Greeting you at home
• Linking with support networks
• Food shopping
• Benefit checks, pendant alarms and blue badge applications

Is there a cost?

No. Our Home from Hospital offers free practical support.

Could you help support this service?

If you feel like you could help us provide practical support to an older person after a stay in hospital. Please do get in touch, as we may have a voluntary position available for you.
