Job Purpose
To provide support to older people in Bradford on Avon and Trowbridge, helping people to access information and advice and to become better connected with their communities, particularly people who are on a low income, socially isolated or find it difficult to access services.
Support volunteers who will be providing a range of practical and other support to older people in the project areas.
Key Responsibilities
As a Community Connector working with older people in Bradford on Avon and Trowbridge, you will engage with individuals in their homes to identify their needs, challenges, and goals through person-centered conversations. Your responsibilities will include providing information, signposting to relevant services, and connecting clients with appropriate resources, such as welfare benefits, social care, and local community services.
You will work closely with local voluntary and statutory organizations, ensuring clients are well-connected to their communities and have access to the necessary care assessments. Collaboration with Age UK Wiltshire services and volunteers is essential, as well as maintaining accurate records and contributing to project evaluations.
Additional duties include attending meetings, identifying service gaps, and supporting the wider objectives of the organization. Flexibility, a commitment to safeguarding, and adherence to Age UK Wiltshire policies are key components of the role.
