The difference we can make

Jane and Fred had been using our home help service for the past four years. They were initially referred to us by the Prevention Assessment Team. Fred had dementia and Jane who has diabetes, was his main carer. Jane needed a bit of extra help at home and so Louise, one of our home assistants, went to their house twice a week to clean, do the ironing and get a shop in.

Eighteen months in and Louise raised concerns at head office that Jane wasn't well and kept having falls. Jane also told her that Fred had caused a small house fire by knocking over a lamp. Our co-ordinator phoned Jane to see how we could help - suggesting that we put her in touch with Pro-active care and the PAT team - to which Jane agreed and we organised.

Louise was due to visit the following Monday. When she used her key code to get in, she found that Jane had been admitted to hospital over the weekend, Fred was on his own and the Care line wasn’t working. Louise checked in with head office and got permission to take Fred to visit his wife in hospital – he’d been very worried about her. We spoke to Jane and sorted out the Care line issue and with her agreement got Meals on Wheels delivered as she was due to be discharged shortly.

Once Jane was home, our co-ordinator visited. Jane explained that she was still struggling to cope since her discharge. With our Information and Advice team, we worked through her concerns - including sorting out a wet room that had been installed the previous year but was unusable as the water was too hot. We also organised an extra hour a week home help support.

Our home assistants love working independently (and setting their own hours) but also appreciate the support and back up of working with Age UK West Sussex. They don't have to look for clients and they get paid at the end of each job by the client directly.
