Community Agent Service - West Sussex Only

Community Agents work out in the community supporting older people to connect with activities and services to improve their wellbeing.

Who is this service for?

This service is for people aged 65 and over who are feeling lonely or socially isolated.

How can we help?

We can call you or visit you in your home to find out what your interests are and what you are hoping to achieve.

We can also discuss any barriers you may have that are preventing you from being more social, these may be physical problems like mobility or transport difficulties or perhaps emotional problems like lack of confidence or worries about meeting new people.

What can we do?

✅ An initial home visit or phone call to discuss your goals and the help we can provide.
✅ Provide information about groups that are available locally that match your interests.
✅ Connect you to activities, services and people in your local area.
✅ Offer information about transport options if required.
✅ Provide support to re-engage socially.

If we find a need and demand for a new group, we will do our best.
