Vacancies - Health Coordinator and Carer/Support roles
Hours: Various hours – temporary, permanent and volunteer
Salary: Between £19,500 - £21,500 pa (pro rata for part time hours)

We have a variety of roles paid (permanent and temporary) and voluntary across our Health services.
Some of our temporary roles are specific over the winter period starting end of November. We have a variety of roles and hours to suit you. We know that work life balance is important so we try to fit working hours to suit your needs as well as matching the needs of our clients. We are looking for people who want to help our elderly and vulnerable clients stay warm and safe and who genuinely care. Whether it’s making the perfect cup of tea, listening to stories or finding the right match for a client, we need you.
Health Coordinators £21,500 pa
You will help coordinate our health services and undertake assessments developing personal plans and matching clients with a relevant member of our team.
Carers – General from £19,500 pa
You will provide older people with low level support for a limited period after hospital discharge or instead of a hospital stay, ensuring they are safe and well in their home. In addition, assessments will need to be carried out, developing personal support plans. You will ensure customers are supported through this period. This may involve practical support such as making a hot drink or snack for a client and just checking in on their everyday wellbeing, which could be as simple as a little chat to brighten their day.
Dementia Support Workers £19,500 pa
You will help create safe and supportive spaces for our members/carers to use our day services. Duties could include assistance in delivering and developing meaningful and stimulating activities as well as providing support to clients with their mobility.
Requirements
You need to be an excellent team player, with good timekeeping – you need to arrive on time, make the time to listen and understand what people need and take time to care. Making sure that those you are caring for are the heart of every decision and action you make.
A driving licence and car may also be required for some of our roles as travel may be involved, but please do chat to us as this is not a requirement for all roles.
