Our Post-discharge Support Service from the Wellbeing & Connections team helps Westminster residents aged 60+ who have recently been discharged from hospital or had a significant change in their health status. If you, or someone you know has recently been discharged from hospital, we can help support the transition back into home life.

How does the service work?

Once a client has been referred to the service, our team will contact the client to arrange an initial home visit. An assessment will take place to determine what the client requires immediately. Our team can assist with services like short-term light shopping, initial assistance with prescriptions, helping with GP contact, and a friendly first point of contact over those ‘settling in weeks’ following discharge.

After a few visits, the client will then be signposted to relevant services both internally and externally. This can include our services like Befriending, Exercise at Home, and Digital Inclusion as well as other charities, council services, and NGOs.

This is a free service.

How do I make a referral?

We accept self-referrals as well as referrals from family, friends, community health workers, and medical professionals.
