Our Hospital Discharge Support service helps us to support the safe transition of hospital patients home after they are discharged.
About the Service
This service operates to assist with the safe transition home following a stay in hospital. It also offers support to people who have been unwell and are socially isolated who would like to find out what is available to them and access services. We offer practical and emotional support to enable them to navigate services from the voluntary, health and social care sector.
The client will be offered a home visit to assess what they may need to support their recovery and improve their health and well-being.
We can offer the following:
• Providing information on a wide range of services and activities
• Ensuring the house is safe and warm on discharge
• Assist with emergency grocery shop
• Organising safety equipment in the home
• Picking up prescriptions and liaising with health and social care
• Encouraging exercise and short walks to gain confidence
• A friendly chat and listening ear
• Making referrals to community-based services
• Assisting people to engage with their local community
