The Hospital Transport and Support Service has been running for seven years, working closely with Pinderfields, Dewsbury and Pontefract Hospitals, dedicated to the safe transport of the Wakefield over 50’s population.
With a fleet of Wheelchair Adapted Vehicles we help to take over 2,500 people every year, giving extra support upon discharge from Hospital. Our dedicated team provide everyone the opportunity for a light meal and a cup of tea whilst settling an individual back into their home, along with a home assessment promoting independence that can provide:
- A shop for essentials
- Contact with Family/Friends
- Any immediate attention in the home
- Connections with the community
- Travel to essential appointments
- Referrals onto other services
- Follow up support and the chance to give feedback
The service runs from 7 until 11 Monday to Friday and 10 – 6 at the weekend. Our team of 5 administrators liaise with Health Care Professionals and previous patients to ensure we are efficient and professional from Hospital to home, whilst our 10 Transport Support Workers continue to spread the caring nature of Age UK Wakefield District around the community.
Throughout our journey as a service we have grown to be an integral part of the discharge process, and continue to take a great deal of job satisfaction from finding new ways to alleviate stress in hospital and strengthen bonds in the community.
Anyone with a Wakefield District GP may be eligible for this free service, enquiries must be referred through a health care professional from the required ward/department.
