Dementia Hub Team Leader

Could you lead and support a team of Dementia Advisers to deliver an effective service to clients?
The Dementia Hub has been formed from key learning and insight gathered through a year-long pilot period of delivery and the first joint Health & Social Care Dementia Strategy commissioned in Sutton. The service aims to offer a central, single Point of Support in the Borough and enable people with dementia and their carers to live well and maintain their independence for as long as possible.
We are looking for a Team Leader to provide supervision and support to a team of Dementia Advisers and service volunteers and to co-ordinate the Dementia Hub. This new role will work alongside a passionate team to develop a growing service.
Age UK Sutton are looking for a supportive and person-centered individual to:

Team lead five part-time Dementia Advisers and service volunteers;

Provide a seamless service that offers consistent and timely access to information and support, including exploration of client needs and triaging as appropriate;
- Build relationships with health and social care professionals and work within the Memory Assessment clinic;

Carry out skilled record keeping and contribute to the monitoring and evaluation of the service; and

Promote the service within all communities to ensure the service is accessed by individuals across the Borough.
