The Hospital Discharge Service is a short term home visiting service to support people aged 60+ who are discharged from Hospital. The aim of the service is to provide a safe and comfortable return home to enable individuals to maintain their independence and reduce re admissions into hospital .
Referrals are received directly from hospital staff and are for clients without a formal care plan/ package.

Tasks includes:
• Shopping
• Light housework
• Ironing
• Companionship
• Meal preparation
• Referrals and signpost
