Stockport Support Hub - Home Support
Having a safe and comfortable home environment is essential for maintaining health and wellbeing and quality of life. Sometimes practical matters get overwhelming, or you may not have the right support networks to turn to for assistance.
We provide practical support to help you live safely and independently in your own home and community. If you are being discharged from hospital, we can help to ensure that you are able to return to a safe home environment.
We also provide experienced and specialist support if you are struggling to maintain your home environment, for example, self-neglect or hoarding.
We will work with you flexibly to provide a wide range of practical and emotional support. This will typically be for up to a month or two, enabling you to feel more confident and move forward independently.
We will also work with you to coordinate access and link you into information, advice and other practical and community resources that can support you going forwards.
