Age UK Solihull is an independent, local charity and a Brand Partner of the national Age UK. We exist to be here for older people when they need our help and support. We are also the lead partner for the Solihull Community Advice Hubs, providing information, advice and support to all adult residents of Solihull.

Over 70 staff and 300 volunteers support our work.

Care Coordinator

Salary: £24,136 per annum

We are currently seeking a  Care Coordinator to join our team. Your main focus will be to identify and work with a defined cohort of patients and holistically bring together all their care and support needs, explore options to meet those needs and develop a single personalised care plan. You will work closely with GP surgeries, primary care professionals and social prescribers. 

If you have excellent communication skills, can empathise with people and provide person-centred support in a non-judgemental way, we would love to hear from you.

We are searching for a dynamic and enthusiastic individual dedicated to improving the lives of older people in Solihull. This role focuses on creating and coordinating engaging activities and programs designed to reduce isolation, promote social interaction and general improvement of wellbeing in older people.
