Personal Assistant
JOB PURPOSE
To provide practical help and assistance to older people enabling them to live independently within their own home and supporting them to access their local community.
RESPONSIBE TO: CSS Operations Manager
LOCATION: Throughout the Rotherham Borough
HOURS: Zero hours contract. Mon – Fri between hours of 9 – 4
SALARY: Scale 9 £11.44 plus mileage and travel time
Roles & Responsibilities
- Service Delivery
- To follow support plans and risk assessments to ensure the safe and effective delivery of the service, by providing household duties, including meal preparation and shopping and prompt Health & wellbeing of the service user.
- Provide a high quality service encouraging independence where appropriate to the service user
- To support the service user to access primary care and support with appointments.
- Support the service user to be less lonely/isolated by supporting to attend community activities and groups and taking them on outings and trips.
- To be proactive as ambassadors of Age UK Rotherham offering information in relation to and providing support to access other Age UK services
- To liaise with appropriate staff re changes or issues regarding their service user.
- Liaise with family members and other professionals as directed by manager.
- Initial planning of cover for own service users during holidays
- Reports and monitoring
- Complete and submit relevant service user information in line with procedures and deadlines to aid the smooth running of the service.
- Provide and receive written information regarding service users conditions and their abilities.
- To continually monitor and review service users to ensure the support plans suit any changing need and alert manager to any changes.
3. Health & Safety
- Ensure the service is delivered in line with each service users individual support plan and risk assessments.
- Report any Health & Safety incidents or issues promptly to the operations manager using AUKR procedure
4. Finance
- Be responsible for handling small amounts of money, collecting appropriate fees for service and issuing receipts whilst following Age UK Rotherham’s financial procedures
5. Personal Development
- Undertake training as required by AUKR and for personal self development.

Assessment Officer
JOB PURPOSE
To carry out comprehensive holistic assessments within older peoples own home, signposting them to relevant agencies to help them maintain their independence. Support in the smooth transition from a hospital stay back to their own home environment.
RESPONSIBE TO: Care & Support Assistant Operations Manager
LOCATION: Galax Business Centre and in the Community
HOURS: 30 hours per week
SALARY: Scale 19 £13.70 per hour plus mileage, pension
- Service Delivery
- Provide a high quality confidential person centred, assessing and support planning service, by making initial assessments, including full risk assessments.
- Provide a high quality 1st point of contact to both service users and professionals.
- To support in the discharge process from a hospital environment by transporting service users home in a safe and timely manner and help them to settle back in.
- Signpost and refer to appropriate agencies.
- Support service users to access primary care services as appropriate.
- Maintain effective partnerships with other professionals to maximise accessibility to all.
- Provide detailed, easy to read support plans and reports using assessment information gathered.
- Regularly monitor, review and case manage individual service provision and support plans to meet the changing needs of the service user.
- Ensure accessibility to appropriate welfare benefits to maximise service user income.
- To be flexible and available to work over a 5 day week between the hours of 09:00 – 18.00 on a rota basis.
- Be flexible to cover at short notice for holidays and sickness.
- Monitoring and Quality Assurance
- Regularly conduct quality checks through agreed quality assurance systems and interviews, both random and in response to raised issues.
- Health & Safety
- Ensure that risk assessments for jobs/activities are followed, maintained and updated.
- Ensure that all services are compliant with all relevant current legislation.
- Other duties
- Keep maintain and provide records and reports as appropriate and as required, using the organisations care management system.
- Liaise and submit relevant documentation on a regular basis to the assistant operations manager to assist in evaluation of the service.
- Comply with the organisation policy and procedures.
- Undertake training as required by AUKR and for personal self development.
This role profile is not exhaustive it will be subject to periodic review and may be amended to meet the changing needs of the business. The post holder will be expected to participate in this process and we would aim to reach agreement to the changes.

Wellbeing Coordinator
Could you be Age UK Rotherham’s new Wellbeing Coordinator?
We are looking for someone who is passionate about communities, with a good
understanding of older people and the challenges that can arise in later life.
This is an exciting opportunity for a skilled person to join our talented staff team
helping older people in Rotherham improve their wellbeing through a
‘Community Circles’ approach.
We are an independent local charity, affiliated to the Age UK network, working
with and for older people across Rotherham. Our work focuses around
what local older people say matters most to them:
- being less lonely
- staying independent
- being and feeling safe
- maintaining physical, mental and social health
- being connected within communities
This project has been funded through The National Lottery Community Fund
for 3 years.
Wellbeing Coordinator
Hours: 30 hours per week
Fixed term contract to 31st March 2028
Salary: AUKR Scale 24, £14.03 per hour, £25,538.28 pro rata per annum
This is an excellent opportunity to take a leading role in the development,
delivery and evaluation of this project. You will use our ‘Community Circles’
approach as a way to improve the mental, physical and social wellbeing of local
people throughout later life.
We hope to reach those who can benefit most including those who are older,
frailer, from BAME communities or have existing poor mental health. This
programme will use both paid staff and volunteers to help people build
personalised support circles to improve their physical, social and emotional
wellbeing.
