Living Well
The Living Well service works with TOG MIND and Positive Steps to deliver an integrated approach in supporting older people experiencing mental health conditions, cognitive impairment or dementia diagnosis to be independent in their own homes by supporting them to identify options, help action their choices and remain engaged in their own community.
The service provides short term (up to 12 weeks) practical and emotional support to assist clients to regain their independence, wellbeing, and confidence, through the provision of a flexible and creatively tailored approach to meet their personal needs, offering initial support by offering options and choices and linking them with appropriate services to support with social engagement activities within their locality. Arranging for practical measures to be put in place that will enable the older person to live in their own home independently and safely for longer.
Support options could include:
• Linking clients into TOG MIND for mental health, wellbeing support and listening space.
• Linking Clients with Positive Steps for benefits and housing support
• Linking Clients and their families to Dementia Support services if appropriate
• Linking clients with social groups and activities in their local area
• Linking clients with shopping services
• Linking clients with cleaning support options
• Installing Key safes and assisting to apply for helpline services
• Providing information on day care options
