It is a collaboration between Age UK Norwich, British Red Cross and Voluntary Norfolk, providing a range of staff and volunteers that are suited to meet the needs of the clients.

The support offered includes a range of tasks, such as:

• Pre and post discharge support
• Arranging practical support for the home/garden
• Arranging adjustments for your home to remain independent
• Ensuring living essentials are there such as food, drink and medicines
• Arranging companionship for those who are lonely, and work with them to become more social
• Setting goals, understanding needs, and working with the client and other trusted partners to ensure the clients needs are met

Please be aware that general support is typically short term, between 2-4 weeks. However, for more complex needs and cases, this can be extended up to 12 weeks, and this is through working together to assess and support. Please be aware that the service does not include personal care such as podiatry or washing, but the team may be in position to source this support for a client where appropriate.
