Our Community Support team offers solutions that can help you during periods of illness or injury, or to adjust to long-term conditions.
Through our practical and emotional support, we can help you to make day to day life a little easier, and avoid small things escalating into big problems.
The service
This service involves regular calls/home visits to understand how you're doing and and worries you may have. We provide a wide range of help and support based on your immediate needs, and thinking ahead to the future, including:
• Games and activities proven to keep your mind active and stimulated
• Physical activites to improve mobility, strength and balance to reduce risk of falls
• Arranging transport to healthcare appointments to support treatment or health checks
• Practical support for home/garden.
• Arranging adjustments to your home for independence
• Support to help you understand care options such as personal alarms/mobility aids
• Ensuring you have living essentials such as adequate food, drink, medicine
• Companionship if you feel lonely or arranging support from people with similar issues
Norfolk and Waveney Community Support Service
Welcome to our new collaborative service, the Norfolk and Waveney Community Support Service (NWCSS). We are looking forward to working with our partners The British Red Cross, and Voluntary Norfolk, where together we will be taking referrals from those who are to be discharged from hospital, or adult social care, and aim to help reduce the risk of them being readmitted and provide support for independent living
