Empowering people to take control of their health and wellbeing by connecting them to non-medical support.

Non medical solutions to worries and problems that are affecting your health

The service takes a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Link workers support existing groups to be accessible and sustainable and work collaboratively with all local partners.

Clients are referred to non-medical ‘link workers’ who give time and help people focus on "what matters to me".

Social prescribing can also strengthen community resilience and personal resilience. It can also help to reduce health inequalities in the county. Our Link Workers help address the wider determinants of health, such as debt, poor housing and physical inactivity. Increasing people’s active involvement, supporting people with long-term conditions (including support for mental health), and people who are lonely or isolated can have a positive effect on wellbeing.

Working with PCNs

We are working with groups of GP surgeries (primary care networks) to provide support to people registered with them.

We are working with Northamptonshire Carers and Serve to provide support to people aged 18 - 65 years in the Wellingborough and District Primary Care Network.

Our seven Social Prescribing Link Workers work with over twenty GP surgeries in the north of the county and are based in GP surgeries.

• We visit people at home or go for a walk with them in their local area to find out how we can help
• We work to empower families so that they can support their relative
• We encourage our clients to join local groups and activities they will enjoy
• We will refer people to other Age UK Northamptonshire services where appropriate
