Our trained volunteers will visit a person's home following assessment for the service by a health professional.

What does the service offer?

This service aims to provide company, reassurance and low-level social support to an older person in their own home. An Age UK Northamptonshire volunteer will visit when support is needed but a family member or friend is not locally available.

• We can offer companionship and reassurance when a delay is anticipated for Primary (GP surgery, nurses etc) or Secondary (Age Well, our own Collaborative Care Team etc) care support
• We can visit to check on someone's welfare
• Available Monday to Friday

Who is it for?

For older people living in Northamptonshire who have been assessed by the county's Remote Monitoring Hub nurse or their GP and meet the criteria for the service.
