Hours of work: 37 hours per week

About the role

As a Coordinator in our Collaborative Care Team you will work in a task centred way primarily with older people who are at risk of hospitalisation or becoming dependent on statutory and/or institutional care.

Working alongside GPs and other Community Health professionals you will be responsible for undertaking comprehensive holistic assessments of older people, usually in their own homes, to create personalised care plans which will assist the person to live well in their own environment, manage their long-term conditions, reduce feelings of isolation, and improve their general wellbeing and quality of life.

You will coordinate and direct the team to carry out identified actions required to meet the needs of older people.

You will promote the work of the Collaborative Care Team and Age UK Northamptonshire throughout Kettering and the local area, and will be responsible for developing and maintaining good working relationships with local GPs, other health professionals and service providers.

You will be responsible for ensuring all patient identifiable information is stored and disposed of appropriately, and that Information Governance and Confidentiality procedures are adhered to.

About you

We are looking for someone who has experience of working with older people and who can demonstrate the ability to listen to others and work in a non-judgemental manner. You will be educated to A level or equivalent, and have a knowledge and understanding of the problems older people face in their daily lives.

By supporting people in the community you will enable them to remain independent in their own home and reduce the risk of them needing crisis support. You will need to be able to absorb information and plan ahead, working in a systematic and organised way.

The Collaborative Care Team Coordinator provides leadership to a small team, helping them develop their skills, and motivating them to provide an excellent service.  If you are motivated by challenge and enjoy seeing problems through to their solution, knowing that your timely actions have improved life for someone else, this is the job for you. A good knowledge of services available to older people in Northamptonshire is desirable. The role covers Kettering and the surrounding areas so you must be a car driver with access to your own vehicle.
