Age Well Coordinator
Location: Wellingborough and district
Based at Waterside House, Station Road, Irthlingborough NN9 5QF
Rate of pay: £22,323.60 pa
Hours of work: 30 hours per week
About the role
We are looking for someone to work in a task centred way, primarily with older people who are at risk of hospitalisation or becoming dependent on statutory and/or institutional care. Age Well gives people the support they need to regain their confidence and have more control over their lives. You will take referrals from GP surgeries and the General Hospitals, enter details on a local computer system and subsequently ensure that all records are accurate and up-to-date.
The role involves visiting clients in their own homes to monitor the service and re-evaluate goals if appropriate. Or you will make telephone check calls to offer short term support on a more intensive basis to monitor the service, as well as making telephone check calls to offer support.
Where identified as helpful and agreed with the patient, you will visit them in their home, taking a laptop to allow the patient and family to participate in a multi-disciplinary meeting, or extended patient review, with health professionals on a Microsoft Teams call or attend desk top reviews with the wider team.
In consultation with the Wellbeing Integration Lead you will play an active role in staff and volunteer recruitment, retention, training and development.
About you
We are looking for someone educated to A level or equivalent, with experience of working with older people and who can demonstrate the ability to work in a non-judgemental manner. You must be able to listen to others and absorb information as well as be able to instil motivation in others. A good knowledge of services available to older people in Northamptonshire is desirable. You must be a car driver with access to your own vehicle. If you are motivated by challenge and enjoy seeing problems through to their solution, this is the role for you.
