Location:   Covering Northampton, based at The William and Patricia Venton Centre

Rate of pay:  £18,096 per annum (FTE £26,782)

Hours of work:  25 hours per week

About the role

We are looking for someone to work in a task centred way, primarily with older people who are at risk of hospitalisation or becoming dependent on statutory and/or institutional care.  With our support people can regain their confidence and have more control over their lives.  You will work as part of a multidisciplinary Age Well Team comprising Adult Social Care, Northamptonshire Carers, Alzheimer’s Society, Northamptonshire Healthcare Foundation Trust to support patients and increase broader choice for their social support. 

The role involves visiting clients in their own homes to monitor the service as well as making telephone check calls to offer support.  In consultation with the Wellbeing Integration Lead you will play an active role in staff and volunteer recruitment, retention, training and development.

About you

We are looking for someone educated to A level or equivalent with experience of working with older people and who can demonstrate the ability to work in a non-judgemental manner.  You must be able to listen to others and absorb information as well as be able to instil motivation in others.  A good knowledge of services available to older people in Northamptonshire is desirable. You must be a car driver with access to your own vehicle.  If you are motivated by challenge and enjoy seeing problems through to their solution, this is the role for you.
