Salary: £23,000 FTE
Hours: 21hrs per week. (including some weekends & bank holidays)

If so, we are looking to recruit a Living Well Support Service Wellbeing Worker to support the existing team. They will work across the community to support patients in the Lancaster and Morecambe Bay areas.

Age UK Lancashire's Living Well Support Service provides support for anyone over 18 years old following a long period of illness or hospital stay. Our service provides essential support to patients who are both at risk of admission and have been discharged from hospital.

In providing this service we are able to:

  • Take patients home directly from the hospital and settle them back at home, ensuring they have everything they need to remain at home.
  •  Provide continued support for up to 6 weeks, to help them retain or regain their independence.
  •  Provide support to those individuals who are at risk of admission.

The successful applicant should have:

  • Experience/knowledge of working with vulnerable or older people
  • An ability to provide a person-centred approach, promoting independence at all times
  • The ability to work well within a team and able to use their own initiative.
  • The ability to communicate effectively across a wide range of people 
  • Able to manage a busy workload,keeping accurate records and assessments.
  • The ability to carry out assessments and keep accurate records
  • A full driving Licence, appropriate Insurance cover and be willing to transport patients in their own cars.
