Get free help to make your advance care plan

  • Age UK Lambeth and other consortium members are offering one-to-one support to help you plan ahead and record your future treatment and care wishes. So, if you become unwell and unable to tell people what you do and don’t want, your loved ones and the professionals involved in your care will know what is important to you.
  • This is a new free service we are delivering as part of the Lambeth Advance Care Planning Consortium.
  • The service is for anyone in Lambeth who would like help to make an advance care plan, including:
    • People with multiple long-term conditions including dementia
    • People with learning disabilities
    • Unpaid informal carers
    • People from Portuguese-speaking communities.
  • We can tell you about the ways you can record your wishes, help you decide which documents you want to use, support you to complete them and share them with the right people. You can choose any combination of these three documents:

 

Referral process

 About the Consortium

  • Lambeth advance care planning consortium is an open network of organisations working together to promote advance care planning across the borough.
  • The consortium brings together a range of local and national organisations including grassroots community groups, specialist charities, services for older people and carers, hospital trusts, Lambeth Council and SE London Clinical Commissioning Group.
  • This service development project is being delivered by: Age UK Lambeth, Ask Erwin, Carers Hub Lambeth, Compassion in Dying, Healthwatch Lambeth, Lambeth Learning Disability Assembly, St Christopher’s Hospice and Stockwell Partnership. It’s funded by the National Lottery Community Fund.
