Lambeth Advance Care Planning Consortium
Get free help to make your advance care plan
- Age UK Lambeth and other consortium members are offering one-to-one support to help you plan ahead and record your future treatment and care wishes. So, if you become unwell and unable to tell people what you do and don’t want, your loved ones and the professionals involved in your care will know what is important to you.
- This is a new free service we are delivering as part of the Lambeth Advance Care Planning Consortium.
- The service is for anyone in Lambeth who would like help to make an advance care plan, including:
- People with multiple long-term conditions including dementia
- People with learning disabilities
- Unpaid informal carers
- People from Portuguese-speaking communities.
- We can tell you about the ways you can record your wishes, help you decide which documents you want to use, support you to complete them and share them with the right people. You can choose any combination of these three documents:
Advance Statement
For recording what’s important to you: We can help you consider your wishes, complete the document and share it with the right people. This may take more than one session. At the moment, due to the pandemic, this is phone-based support.
Advance Decision
To record medical treatments that you do not want in particular situations: Compassion in Dying’s information line can support you to complete an Advance Decision. This is phone-based support
Lasting Power of Attorney for Health and Welfare
To give someone the legal power to make decisions on your behalf: We will help you complete the document and share it with the right people. This may take more than one session. At the moment, due to the pandemic, this is phone-based support.
