WHAT WE'RE ACHIEVING
Over the period April 2023 – March 2024 we supported 4781 people aged 16-99+ across 6146 'cases' of support. 92% of the people we supported achieved a successful outcome.

WHO WE'RE REACHING
Of the people we supported: 40% of people lived alone, 90% had one or more health condition and 54% were from a BAME group.

'CONNECTING' FUNCTION
We're bridging the gap between health and statutory services and the wealth of support across the borough. Last year we connected people to 575 unique activities and services - careful matches with support and follow up to make connections work.

WHAT OUR CLIENTS SAY
We ask every single client what they think of us, after every piece of support. 94% of people told us they would be 'likely' or 'extremely likely' to recommend us.
