Hospital Discharge Service

If you have been admitted into Northwick Park hospital and are now ready to be discharged, you may be offered our FREE discharge Support Service.

Our team will escort you home safely by taxi and help you get settled. 

They will also carry out some small household tasks if needed:

• Helping with essential shopping.
• Changing your bed.
• Supporting with post.
• Preparing a light snack and a drink.

and if necessary offer you a daily call for up to three days following discharge.

Service for people 60+ living in the boroughs of Harrow and Brent.

Working with Harrow Carers to deliver the discharge service at Northwick Park Hospital.
