Supporting your discharge from Northwick Park Hospital. FREE service for people 60+ living in the boroughs of Harrow and Brent.

Discharge Support Service from Northwick Park Hospital

If you are being discharged to the boroughs of Harrow or Brent, one of our friendly support workers will escort you home and support you to get settled by:

• Helping with essential shopping
• Changing your bed
• Supporting with post
• Preparing a light snack and a drink

Then if you wish they can call you for two to three days after discharge to check that all is well.
