Acute Frailty Event

Julian Lloyd spoke at September's Acute Frailty Event about why we are proud to offer a FREE Discharge Support Service for individuals 60+ in Hillingdon, Harrow and Brent.

This initiative ensures a smooth transition from hospital to home, promoting well-being and independence

𝗪𝗵𝗮𝘁 𝗪𝗲 𝗢𝗳𝗳𝗲𝗿:
• Essential shopping assistance
• Changing bed linens
• Managing post
• Preparing a light snack and drink

Our support workers escort patients home and help them settle in. We then provide follow on support in the community, providing holistic review working with the patient to set goals and optimise rehabilitation.

𝗪𝗵𝘆 𝗧𝗵𝗶𝘀 𝗠𝗮𝘁𝘁𝗲𝗿𝘀:
• 𝗥𝗲𝗱𝘂𝗰𝗲𝘀 𝗥𝗲𝗮𝗱𝗺𝗶𝘀𝘀𝗶𝗼𝗻 𝗥𝗮𝘁𝗲𝘀: Prevents complications and reduces hospital strain
• 𝗣𝗿𝗼𝗺𝗼𝘁𝗲𝘀 𝗜𝗻𝗱𝗲𝗽𝗲𝗻𝗱𝗲𝗻𝗰𝗲: Empowers seniors in a safe environment
• 𝗖𝗼𝗺𝗺𝘂𝗻𝗶𝘁𝘆 𝗪𝗲𝗹𝗹-𝗯𝗲𝗶𝗻𝗴: Ensures our elderly receive the care they deserve
• 𝗖𝗼𝘀𝘁-𝗘𝗳𝗳𝗲𝗰𝘁𝗶𝘃𝗲 𝗛𝗲𝗮𝗹𝘁𝗵𝗰𝗮𝗿𝗲: Reduces hospital readmissions and associated costs
