'I had been feeling a bit low having been in hospital and the navigators visit with the food has really cheered me up.'
Patricia has recently been discharged from hospital during COVID-19, and because she was classed as ‘High Risk’ she had to shield at home. This meant she was unable to go out to collect her medication and get her food shopping.
Patricia’s daughter was also unwell and shielding, so was unable to do things for her mum. Patricia phoned us as she was worried she would not get her medication or food, the Hospital & Community Navigation Team (HCNS) were able to help.
The team organised the collection of her medication, as well as delivery of emergency food parcels to her home, and she was delighted:
“I had been feeling a bit low having been in hospital and the navigators visit with the food has really cheered me up.”
To help Patricia with her independence, the HCNS Team provided her and her daughter with details of alternative local and supermarket support options and information on how to register as shielded through the government website.
Patricia’s daughter was over the moon with the amount of help that her mother was receiving saying:
“I organised a food box to be delivered from a supermarket using the contact details we provided – what a great service!”
