Our Help Team work to provide much needed support to individuals and families, spanning many different aspects of later life. Their guidance enables people to feel safe, comfortable and supported. Much of this role involves creating a space for people to speak freely about any difficulties they are having before going on to find the ways in which we can help. Chatting to a friendly, compassionate person when struggling with confusing systems, financial worries, loneliness or ageing in general can make such a difference.

Hospital to Home Coordinator

Our Out of Hospital Team support people following a discharge from hospital, ensuring people feel settled, safe and comfortable after their time in hospital. This role will work within the Gloucestershire Royal and Cheltenham General hospitals, helping people to address any potential worries or difficulties surrounding their return home. Making the transition home as comfortable as possible can really impact how well someone can continue to live both independently and confidently in their own home.

Age Friendly Guidance Manager

We’re looking for someone to lead our amazing Help Team! The team provide information and guidance to individuals themselves as well as family, friends and carers to ensure people are informed and confident as they go through later life. Throughout the team a wealth of information is held on the support available around the county, sharing this knowledge and making sure people are able to access the help they need is a hugely important aspect of this role. They are often the first port of call when people face difficulties, with the many different ways this team support older people in Gloucestershire, this opportunity would be really suited to someone looking to really make a difference to the lives of older people in Gloucestershire.
