Age UK Gateshead are working to support the Queen Elizabeth Hospital and other health and social care professionals to deliver this bespoke service doing things such as:

Over 1500 welfare calls each month
Providing direct referral pathways into services such as benefit advice, social opportunities, mental health support, bereavement services, dementia specialist services, occupational therapy and much more
Proving long term support when necessary to enable confidence within an individual, often exceeding an average of 12 hours support per referral.


Our hospital to home service will arrange for a support worker to meet you at your home after you have been discharged from hospital.

Our support workers are there to assure your home is a suitable as it possibly can be for the best possible recovery and quality of life.

When in the house our support workers will assess the environment looking to see if some of these essentials are in place such as:

Food
Heating
Electricity
Water
Home Adaptations
Care Packages
And more
After this assessment our support workers will provide the necessary support in that instance. After these interim fixes our support workers will assure longer term services are in places before stepping away- but will continue support by welfare calls.
