ICan

ICan is a free, confidential support service available to Enfield residents aged 18 and over, who have a long-term health condition. This could also include diabetes, dementia, stroke or a having had a fall. We also work with people who are socially isolated or need end of life care.

We aim to enable you to remain independent, and boost your wellbeing. This could be through providing advice on lifestyle or healthy living choices, access to benefits or housing advice, or information and support on managing your health conditions. We are also here to help you build skills, friendships, and connect to others.

The service is commissioned by the London Borough of Enfield and is led by Age UK Enfield.

What we do

We can work with you one to one (via phone, email or in person) to address any concerns you may have. We will work together to set a plan of action, according to what is most important to you. This could be offering information or advice, or supporting you to access other services.

We also offer:
Peer support groups - Stroke Cafe, Diabetes Group, Memory Groups
Health Information and Awareness Sessions
Social activity groups - Tea & Chatter

How Does It Work?

Our navigators can help in a number of ways:
- helping access information and advice (including benefits or housing advice)
- helping access to health and social care services
- providing advice on lifestyle, or healthy living choices
- helping you to meet new people or reconnecting with their community
- building your skills and confidence or volunteering
- assisting in finding places to go and things to do
- assisting in understanding and self-managing your health conditions

We also deliver monthly group sessions, including:

• Peer support groups (Stroke café, Diabetes Group, Walks and Talks)
• Health information and awareness sessions
• Social activity groups – Tea and Chatter
