We are funded by North Central London Integrated Care Board (NCL ICB) to support people with their health and wellbeing, and we work closely with North Middlesex Hospital Community Services Proactive Care team. You may have been told that you have the condition healthcare professionals refer to as frailty. As people age they can become weaker and more vulnerable. Small events such as an infection or a fall can lead to a dramatic change in health resulting in a hospital admission and a lack of independence. We will look at ways to reduce hospital admissions and help you navigate services and correct support. We can work anyone aged over 50, that may be living with complex health and social needs.

Health & Wellbeing Navigators can work with you, one to one and can help by:

Helping you understand and manage your diagnosis or health conditions
Help you to build coping strategies
Help to obtain welfare benefits and travel concessions
Help with planning for later life (information re lasting power of attorney, wills and advanced statements)
Help to access assistance from other organisations
Understanding assisted technology options
Referring to other relevant services and sources of support e.g. occupational therapy, fire safety visits and much more.
