We are commissioned by North Central London Clinical Commissioning Group (NCLCCG) to offer a range of preventation services. These include:
Fall Stop Service
Post Diagnostic Memory Support (Memory Care)
Health& Wellbeing (previously known as Moderate Frailty)
Our team of Health & Wellbeing Navigators work across all the contracted areas to deliver the services. Our work includes working closely with Enfield Memory Service, St Michael's Falls Clinic, BEHT Community Services Anticipatory Care, GPs, local authority and community health services..
We can work anyone aged over 50, that may be living with complex health and social needs and we work together to identify your needs and put together a plan that will help you:
Reduce the risk of falls
Understand your dignosis or health condition
Access support with everyday tasks
Reduce the risk of hospital admission
Feel in control of your life
Reduce the risk of social isolation or loneliness
Connect to others in the community
Manage your health needs
Maximise your income through obtaining welfare benefits
What we will do
If you are referred directly to us, we will undertake a comprehensive assessment, which includes a health, mobility and medications, welfare benefits, and a discussion about wishes and preferences for your future care. We will summarise your outcomes and this can be shared with other relevant professionals if you feel it would benefit any ongoing support.
Avoid Hospital
You may have been told that you have the condition healthcare professionals refer to as frailty. As people age they can become weaker and more vulnerable. Small events such as an infection or a fall can lead to a dramatic change in health resulting in a hospital admission and a lack of independence. We will look at ways to reduce hospital admissions and help you navigate services and correct support.
How We Help
The initial assessment will help identify your wishes, from there we can help you to access support, and help you to maintain your independence and manage your conditions. This could include encouraging a healthy lifestyle; getting you connected to social or leisure/ recreational activities; advice on obtaining welfare benefits, travel concessions, later life planning and connecting with others via groups.
