Age UK East London assists people home from hospital and provides practical and emotional support in the following weeks.

Who is this service for?

This service is aimed at residents of Hackney and is funded by the North East London ICB

What is this service for?

Based out of Homerton Hospital, this service assists people in getting home from hospital. Age UK East London will carry out safety checks in the home, give advice in practical and wellbeing matters, and visit regularly for a period of six weeks after discharge.
