Assessment Officer – Keep in Mind (KiM) Service

Location: Doncaster

Hours: Part-time (21 hours per week, Monday to Friday)

Salary: £22,880 per annum (pro rata)

Are you ready to make a real impact? Age UK Doncaster is looking for a flexible and skilled Assessment Officer to join our Keep in Mind service. Keep in Mind will be delivered by a network of Delivery and Support Partners. It brings together collective expertise to achieve a whole systems approach to Community Therapy and Support Services. This role involves supporting people living with dementia and their carers or families to access essential networks and information.

Key Responsibilities:

  • Holistic Assessments: Conduct strengths-based initial assessments to identify needs.
  • Support Planning: Co-produce appropriate and proportionate support plans.
  • Guided Signposting: Help individuals access internal and external support services as needed.
  • Accurate Record Keeping: Maintain up-to-date and accurate information across all systems.
  • Communication: Work closely with the individuals we support, their loved ones, and statutory agencies.
  • Collaboration: Assist in evolving and embedding new approaches by working with colleagues on projects and learning opportunities.
  • Impact Measurement: Achieve agreed targets, report on key performance indicators, and document success stories through case studies and life stories.
  • Teamwork: Contribute to service continuity by working closely with the wider team.

What You’ll Bring:

IT Proficiency: Strong skills in Microsoft Office and confidence in using databases to manage information accurately.

Passion for Community Work: If you have excellent interpersonal and communication skills, experience in social care or health, and are seeking a rewarding role, we’ll provide the training to set you up for success.

Empathy and Motivation: A natural ability to connect with others and make a positive impact in their lives. Active listening skills and experience in holding motivational, person-centered conversations are essential.
