Survey for people who have had cataract treatment
Healthwatch Derbyshire are carrying out a survey to find out peoples experiences of care and treatment for cataracts.
They want to find out what works well, and where things could be improved, so that patients can be part of the design and delivery of services to meet their needs.
The closing date is Sunday 2nd September 2018.
Individuals who complete the survey can enter our prize draw for a £30.00 Marks & Spencer gift voucher.
