Service creating and providing support plans with GP practices.

The Older Persons Integrated Care Pathway is an innovative service which works within selected GP practices in Newport to identify older people who are invited to participate in the project.

They are then visited at home and offered the opportunity to create a Stay Well Plan which focuses on all aspects of their life to help them maintain their health, wellbeing and independence.

If you are interested in the service ask your GP surgery if you can be referred and we will be in contact as soon as possible.
