Returning home after a stay in hospital can be difficult, especially if you live alone. We can support older people aged 50+ to settle back into their homes.
The service is available in Blaenau Gwent, Monmouthshire and Newport.

How we can help

When you return home from hospital, we can support you in the following ways:

• Confidence to return home safely.


• Reducing delays in discharge.


• Empowering individuals to become more independent.


• Lacking loneliness and isolation.


• Emotional support to help settle you back into your home.


• Working with you to shop for essentials.


• Support and prepare light refreshments.


• Housing issues or adaptations around the home.


• Signposting and onward referrals to other relevant services.

The service is free and confidential. Our involvement does not affect your rights to any other services and is alongside these. This includes any support you're receiving from social services or other agencies.

All our staff are DBS checked and carry identification. This will be shown to you during your first meeting.

Monmouthshire - Monday to Sunday
Newport - Monday to Friday

A person may be referred to the service by a:

• Social services


• Family member


• Friend
