Dementia Wellbeing Connector Ceredigion

We are looking to recruit a self-motivated, hard working team member to join our exciting new service that supports people living with Dementia and their carers.The service aims to provide a full wrap-around support that will assist in navigating individuals’ journeys and increase overall wellbeing. This is a part time hybrid role, working from home and from our Aberystwyth office and with travel throughout Ceredigion to client homes. The contract is fixed term (28 hours a week, FTE £24,700, actual £19,760) until end of May 2027, with potential for an additional two-year extension.
