Croydon's Health and Care Partnership

Age UK Croydon's Personal Integrated Care programme is one of a number being rolled out across the country. This service sees branches of Age UK partner with local NHS trusts and councils to provide person-centred support to vulnerable older adults with complex health and social care needs.
