We are seeking someone with excellent communication, interpersonal and leadership skills, able to form positive working relationships with a wide range of people. Organised and accurate coupled with the IT skills to confidently record clients and referrals on to digital databases and systems, such as CharityLog and Strata. Finding opportunities for some of the most vulnerable in our community, to age well in their homes and communities.

Outcomes
Working within the hubs and across the community of mid-Cornwall, this role will support and encourage a small team of Hospital to Home Support Workers to empower and support older people out of hospital and back into their home and local community.
You will work with the organisation to create a feeling of connectiveness with peer contact within your team, hosting team get togethers along with larger organisational meet ups. In this role, you will also seek to improve the quality and growth of the service, actioning compliments, issues and complaints from individuals, networking and promoting the service, and developing and maintaining good working relationships with the community and local organisations with the aim of ageing well and living life better. Our charity is agile and flexible, we do whatever is needed for the right outcome at the right time. On occasion, the post holder will be required to be fully involved in the delivery of the service, completing assessments and providing cover within your area.

Background
Since 2022, Age UK Cornwall & The Isles of Scilly, Partners (including Chaos Group, Volunteer Cornwall, Cornwall Neighbourhoods for Change, and others) and Integrated Care Board have been working together to improve access to services, with the aim of reducing demand on acute services. Hours of Work: 24 hours per week. Working pattern to be agreed.
Salary: £15,784.22 per annum (£24,334 FTE)
Place of Work: The role is primarily based at home with the requirement to work within Clients’ homes and occasionally at our Truro Head office or community hubs. The area to be covered is mid-Cornwall including Truro, although support to other areas may be occasionally required, e.g. Newquay, Bodmin, St. Austell Clays. Due to the nature of the work, travel is an essential part of the role therefore a driving license and access to a car is essential. Your car insurance must cover business use.
Duration: Fixed term for 12 months
DBS: This role is subject to an Enhanced DBS check with Adult Barred list check.
