Hospital to Home Support (Newquay & St Austell)

Reports to: Hospital and Home Service Development Lead

Place of Work: St Austell Hub, PL25 5AE and Govenek Hub, TR7 1JR, with the requirement to work at different locations and venues. Due to the nature of the work, travel is an essential part of the role therefore a driving license and access to a car is essential. Your car insurance must cover business use.

Salary: £15,784.22 per annum (£24,334 FTE)

Hours of Work: 24 hours per week, with possible weekend work. Working pattern to be agreed.

Duration: Fixed term until 31st March 2025

About the role:

In this role you will enhance the lives of older people by offering a high-quality service within their homes, hubs, and hospital. You will be responsible for the delivery of practical support and pro-active co-ordination in accordance with an individual’s wishes and circumstance. You will offer a user-focused service, ensuring good practice, promoting independence, confidentiality, and integrity at all times. You will also complete holistic assessments within community hospitals and a persons’ home and provide support to those who attend groups for social activities and support. This may include the Active Living Hubs.

About you:

You will have:

- Experience in practical support, including cleaning, shopping, food preparation and companionship.

- Excellent communication skills with the ability to form positive and constructive working relationships with a wide range of people including:

  • individual clients and their families,
  • other staff and volunteer colleagues,
  • community hospital and social care staff and
  • other charitable organisations.

- Good IT skills with the ability to navigate databases and access client information via a safe and secure file sharing and transfer system.

- Be able to travel independently to clients’ homes, our Hubs and community hospitals.

- Be motivated to make a real difference in the quality of people’s lives.

- A friendly and caring nature with a commitment to understanding the needs of the people who use our services, and a willingness to learn; with an absolute commitment to reaching our standards in training.

This role is subject to an Enhanced DBS check with Adult Barred list check and undertaking necessary training.
