Community Outreach Link Worker

We are seeking people with experience of developing personalised support to apply for the role of Community Outreach Link Worker. As part of the Kernow Community Gateway Team, you will respond to referrals to offer adults a Personalised Support Plan with a focus on “What Matters” to the individual. This conversation will draw upon an individual’s strengths to actively explore the self-management of their Health and Wellbeing, allowing individuals to navigate their way into support provided by both the Statutory and Charitable sectors, such that they feel empowered to make their own decisions regaining control of their life.

You will determine the most appropriate method of support; this could be either a telephone conversation or where barriers exist conducting a home visit. Access to a car is essential.

This role would suit someone with excellent communication skills with the ability to actively listen, empathise with people and provide person-centred support in a non-judgemental way. You will be able to inspire trust and confidence and have the skills to devise appropriate strategies, motivating others to engage in advice and support services, and reach their potential. You will be confident in your computer skills, able to adapt to bespoke case management systems, with the ability to prioritise and work under pressure, to tight deadlines, with accuracy and significant attention to detail.

Work Base: Your base will be the Falmouth Hub with a requirement to attend meetings in the office at Boscawen House Truro or other placed based venues throughout West Cornwall. This forms the basis of the role and will be agreed by discussion. The role has reach across the West Cornwall area with a balance between working virtually and reasonable travel for in person meetings.  There is some flexibility for the post-holder to work from home. You must have access to a car.

Salary: £21,831 per annum (£26,925 pro-rata)

Hours of Work: 30 hours per week, Monday to Friday

Duration: Fixed Term Contract for 12 months
