Hospital to Home Report - Jan to March 24

Age UK Cornwall and the Isles of Scilly, Volunteer Cornwall, The Chaos Group, and range of local Community partners are providing Hospital to Home Services in community hospitals throughout Cornwall.

We are extremely proud to work alongside NHS staff, local community hubs and care providers. Our aim is to deliver tailored support, which puts the person first, to individuals and families who need help following discharge.

The following report provides an overview of our Hospital to Home Services between Jan to Mar 31st 2024.
