We offer a holistic array of services support and activities for people living with dementia, and those that care for them, at all stages of the dementia journey.

Dementia & Me Coordinators

Our coordinators can support you from pre-diagnosis onwards. They can visit you at home to help you arrange a plan of support for every point of the dementia journey.
They offer emotional support and advice around diagnosis and the difficulties arising from dementia symptoms.

Our Coordinators are split between the GP surgeries - so it will depend on which GP surgery you attend as to who your coordinator will be
