If you live in the borough of Camden and are living with memory loss or dementia our Dementia Wellbeing Service can provide flexible support to you, your family or carers.
Practical and emotional one to one support.
The Dementia Wellbeing team provide practical and emotional support to meet your needs. The service aims to support people to live independently and safely at home for as long as possible, stay socially engaged, and carry on doing what they love.
Following an initial home visit they’ll work with you, your family or carers to produce a personalised wellbeing plan. This plan will focus on the issues that you think will make a difference
The team can work with you for 3-6 months and this can include weekly, bi-weekly or monthly visits, as needed. The service also has trained volunteers who can provided additional support as needed.
We can support you with:
• Developing a personalised wellbeing plan
• Finding other Age UK Camden services to support you
• Implementing systems to manage your post/form filling
• Meter readings and informing utility providers
• Navigating Camden Council and other Camden services - repairs, council tax
• Keeping active at home
• Care reviews
• Monitoring your circumstances and flagging up safeguarding issues
• Help you stay connected to loved ones, family and friends
• ‘Outings with a purpose’, eg accompanying you on a shopping trip to buy a winter coat & presents
• Increasing opportunities for social engagement, joining group activities and connecting with community organisations
Dementia friendly services – making a difference
We strive to be a dementia friendly organisation. We treat people living with dementia as individuals rather than focusing on their illness or on any abilities they may have lost. We also provide support, advice and information for carers, families and friends, as well as to other organisations and professionals. Our contact number for enquiries is 0208 103 3991.
Our Dementia Wellbeing Service works with other Age UK Camden services e.g. Health and Positive Living Hubs, Information and Advice teams, Befriending and Care Navigator and Social Prescribing - to provide holistic support and make a real difference.
Referrals
Referral Criteria:
- Age 55 plus and living in Camden
- Has concerns around memory or diagnosed with mild cognitive impairment or dementia

Partnership and Engagement
Our specialist Dementia Services work with a variety of organisations such as Surma Centre, Great Camden Minds, Dementia Action Alliance, Wellcome Trust, Camden Memory Service and British Somali Community Centre
Great Camden Minds is a Dementia Engagement and Empowerment Project where people with dementia meet to discuss reducing the stigma of dementia, have a say in services and policies relevant to people with dementia and get peer support.
